Blended Case Management for Children with Mental Health Needs

 

Blended Case Management offers assistance to advocate for, locate and coordinate services that address the unique needs of your child.

Blended Case Management:

  • Supports families of children, teens and young adults (up to age 21) with autism or a behavioral health diagnosis in navigating complex support systems
  • Assists families with needs spanning all areas of the child or adolescent's life, including medical, mental health, educational, social and vocational issues

Services take place in the homes, schools, and other locations throughout Erie County.
